Includes:

  • Review of the current healthcare situation

  • Identification of immediate concerns and priorities

  • Guidance on next steps and available support options

  • Determination of whether ongoing advocacy would be helpful

This consultation is not required to continue services, but often provides immediate relief and direction.

Hospital Discharge & Care Transition Support

Designed for one of the most critical moments in healthcare.

Includes:

  • Review of discharge instructions and follow-up plans

  • Medication reconciliation and education

  • Home care or facility transition guidance

  • Coordination of follow-up appointments

  • Written summary for families

Ideal for hospital-to-home or hospital-to-facility transitions.

Hourly Patient Advocacy & Care Navigation

Best for short-term or situational support.

Services may include:

  • Medical appointment preparation or attendance (virtual or in-person)

  • Clarifying diagnoses, treatment options, and care plans in plain language

  • Medication review and organization (education only)

  • Communication support with healthcare teams

  • Care coordination across providers or facilities

Billed in 30-minute increments.

Ongoing Family Advocacy (Monthly Retainer)

Best for families managing ongoing or complex care needs.

Includes:

  • Up to 8 hours of RN advocacy and navigation per month

  • Ongoing care oversight and coordination

  • Regular family check-ins and updates

  • Priority scheduling and response times

  • Secure documentation and care summaries
